Apple Physical Therapy is proud to be a preferred provider for most insurance plans. We also bill auto insurance if your injury is due to a motor vehicle accident. Though you are able to receive physical or massage therapy services without a prescription from your physician, please keep in mind that your insurance carrier may not pay for physical or massage therapy services without one. A physician referral is required to receive occupational therapy.

Once you provide us with your insurance information, we will contact your insurance company to get an estimate of your rehab benefits. If your insurance provider requires a co-pay, deductible or co-insurance payment we will ask for payment at the time of your visit. We accept cash, check or credit card payments.

If you DO NOT have insurance or a third party is responsible for payment of your injury, please let us know.
